Community Support Coordinator/Case Manager Full-Time
Position description: The Community Support Coordinator coordinates and provides support to people receiving services from Options and works as part of a team. Each Community Support Coordinator has a small caseload and must maintain a flexible work schedule based on the needs of individuals they support, based on the office hours of 9-5 Mon-Fri. Occasional evening and weekend hours required. Applicants must have an insured vehicle and a good driving record. The current opening is on a team primarily supporting adults on the Autism spectrum. Due to on-call responsibilities, applicants must live with-in 30 minutes of Madison or be willing to relocate.
Duties and Responsibilities:
Develop and implement individualized support plans
Hire, train, and supervise direct support staff
Provide direct support and personal care, as needed
Convene and run regular household meetings
Assist people supported to manage personal finances
Maintain accurate and timely documentation of time spent and log notes on behalf of people supported
Coordination of medical care for people supported
Work cooperatively with family, friends, and other service providers
Participation in agency on-call rotation (with additional compensation)
Candidate Requirements:
Must be 18 years old
Valid driver's license, insured car, and good driving record
High School or equivalent
Successful clearance of background checks
One or more years' experience working with adults with IDD
Other Desired Knowledge/Experience:
Post-secondary education
Professional verbal and written communication skills
Proficiency in basic computer programs: word, excel, PowerPoint, zoom
Experience working with individuals on the autism spectrum helpful but not required
Salary: $50,000.00 per year
